Payroll Deduction Membership Eligibility
By acknowledging this document, the Bowling Green State University (BGSU) employee grants the BGSU Payroll Office and BGSU Office of Recreation permission to deduct pay from each paycheck in the amount specified to this agreement. All memberships are subject to verification for proper eligibility and purchase price. The Office of Recreation (RecWell) reserves the right to adjust and/or correct eligibility, fees or expirations of the membership(s) purchased. Accurate information and required documentation are necessary to receive appropriate membership fees and discounts. Members are required to notify RecWell of any eligibility changes. False/inaccurate information or failure to notify RecWell of any eligibility changes may result in loss of membership and may result in loss of future access.
Membership Fee
- Membership fees are non-transferrable and non-refundable.
- Members are responsible for reviewing pay stubs/bank statements to verify charges.
- Membership fees are not based on facility usage and will be charged/withdrawn regardless of use of the facility.
- Membership rates are subject to change. RecWell will notify all BGSU faculty/staff members in advance of a price increase.
Student Recreation Center (SRC) Payroll Deduction Membership Policies
- To qualify for payroll deduction, the member must be a current BGSU employee (paid over 12-months), and/or a spouse or dependent of a current BGSU employee paid over 12-months who is also enrolled in payroll deduction.
- The BGSU Payroll Office deducts from the employee’s pay in the amount specified to this agreement.
- Membership rates are subject to change at the end of the 12-month period with month-to-month payments continuing at the adjusted recurring rate unless cancelled by the member.
- Breaking the contract or cancellation of the membership results in the forfeiture of the SRC membership at the discounted rate.
- Early cancellation may result in cancellation fees.
- There is no cancellation penalty if the primary membership has met or exceeded twelve (12) months.
Change Requests
- Change requests included, but not limited to, memberships, programs and services are accepted at any time.
- To alter an existing agreement, the BGSU employee must re-submit the “Payroll Deduction Registration Form” found on the RecWell Membership webpage.
- Change requests will take effect on the pay cycle following the approval of the change form, so long as the request is received at minimum ten (10) days prior to the employee’s next scheduled deduction date.
Spousal and Dependent Memberships
- Spouse(s) and dependent(s) must be enrolled with a BGSU employee to qualify for payroll deduction.
- Spouses qualify to be added to BGSU payroll deduction if legally married to the BGSU employee, as defined by the state of Ohio.
- Child(ren) 15 and under must be enrolled with a parent/legal guardian’s membership. Patrons 16+ qualify for dependent pricing only if a parent/guardian holds a primary membership of the same type.
- Patrons 18+ must purchase a primary membership.
- Spouse(s) and dependent(s) that wish to join the Student Recreation Center (SRC) without affiliation to a BGSU employee do not qualify for payroll deduction and are encouraged to purchase a membership and pay via cash, check, or credit card.
Membership Cancellation
- A BGSU employee must be the primary passholder. If the BGSU employee cancels a payroll deduction membership, subsequently, any spousal/dependent memberships are also cancelled.
- Payroll deduction memberships are continuing memberships with a minimum of a 1-year contract term. Early cancellation may result in a cancellation fee.
- Payroll deduction memberships must be cancelled by submitting the “Payroll Deduction Cancellation Form” on the RecWell website at least ten (10) days prior to the next scheduled deduction date. Failure to do so makes subsequent deductions non-refundable.
- Cancellations must be confirmed via email by the Coordinator of Member Services prior to taking effect.
Suspensions/Terminations
RecWell has the right to suspend or terminate SRC memberships for violations of SRC policies, failure to pay membership fees, change in eligibility, or actions which violate any federal, state, or local laws.
Transferring Accounts
SRC memberships and services are non-transferable. Membership privileges are to be used only by the person to whom the membership was issued.
Membership Dates and Facility Closures
RecWell may close the SRC for maintenance and other purposes throughout the year. Additionally, the SRC may close or reduce hours during university breaks and university observed holidays, as well as during inclement weather. Changes to facility hours are posted on the website, social media accounts, and inside the facility. The membership fee has these dates factored into the price. RecWell does not reimburse membership fees due to closures for the above reasons.
Inclement Weather and Other University Closures
During periods of severe inclement weather, public emergency, or other crisis, university officials may announce, through the BGSU electronic alert system or through the local news media, that some or all of the university’s offices and facilities are closed for part or all of the day. RecWell follows university policy and modifies SRC building hours, as needed.
