A&S Student Travel Notification Form
The A&S Student Travel Notification Form is required for all unit-affiliated travel that involves undergraduate or graduate students, regardless of academic credit. The form should be completed for activities that require students to leave the BGSU campus and involve the organized or sponsored transportation of students from the campus to the off-campus location of the organized event or activity. This includes trips/excursions that are:
- Organized or sponsored by the academic unit or program
- Required for a course, including course-related field trips or site visits
- Organized or sponsored by a student organization within the unit or program
As with the University’s Student Travel Policy, the College does not require notification for “students participating in student-teaching, internships, practicums, co-ops, observations, or research, unless the research/observation is organized by a University employee.”
Dates and times of course-related field trips and off-campus activities should be included in course syllabi. Please note that student participation during University recesses is considered voluntary. No off-campus activities of any sort should be scheduled during exam week.
If an activity or travel requires students to miss other classes, they are to be reminded that it is their responsibility to make up work in all classes missed during absence. Students should confer in advance of the tour with all instructors whose classes will be missed (see BGSU Academic Charter B-I F. 3 & 4. "Class Attendance" and “Exceptions to Class Attendance Policy”).

Notification Process
Step 1: For travel that involves overnight stays or outside of a 50-mile radius of the Bowling Green campus, you will need to complete the Office of the Dean of Students’ Travel Registration Form. If using this form, you do not need to complete the A&S Travel Notification Form.
Step 2: Complete the A&S Student Travel Notification Form. Preferably, this form should be completed by the end of the first week of classes, or at the latest, at least 14 calendar days before the commencement of the off-campus activity.
Step 3: Distribute and collect Liability & Release Waiver forms from student participants, if applicable. If travel is part of a course requirement, the Liability & Release Waiver is NOT required. Voluntary or optional off-campus activities, even those associated with a credit-bearing course, require the Liability Release Waiver.
