Student health requirements
All occupational therapy doctoral students must complete and submit health requirements for admission into the program and before they begin clinical experiences. See BGSU’s vaccine information as it applies to OTD students in clinical education experiences.
Must be submitted after admission
All students in the BGSU OTD program are required to have health insurance. You must submit proof of health insurance prior to entering the program. If you are a dependent and on your parents’ insurance, you must provide documentation from your health insurance company.
Students have the option of purchasing university-sponsored medical insurance.
Students enrolled in the BGSU OTD program are required to undergo background checks prior to entrance into the program. An additional background check, drug test and fingerprinting clearance may be required before beginning clinical experiences in year two of the program.
Applicants should be aware that a prior criminal background could restrict the ability to obtain professional state licensure. Acceptance into the OTD program does not imply or guarantee that a student will be able to obtain such licensure.
- Copy of valid driver’s license or state identification
Must be submitted before orientation
- Proof of at least one Hepatitis B vaccination of the 3-dose series or a positive titer
Must be submitted before first clinical experience
- Full 3-dose series of Hepatitis B vaccination with proof of positive titer
Must be submitted within first academic semester
- Proof of influenza vaccine (required annually)
- Proof of Measles, Mumps, Rubella (MMR) vaccine – two documented vaccinations or positive titer test
- Proof of Tetanus, Diphtheria and Pertussis (Tdap) vaccination within the last 10 years
- Proof of Tuberculosis (TB) test – annual 2-step PPD required or negative Quantiferon Gold test accepted. Positive tests require submission of a negative chest X-ray.
- Proof of Varicella (VAR) vaccine – two documented vaccinations or positive titer test
- Completed physical examination form
- Basic Life Support (BLS) certification by the American Heart Association – must be current
- OSHA certification – annual course
- Bloodborne Pathogen certification – annual course
- HIPAA certification – annual course
BGSU OTD does not provide waivers for immunization requirements listed above.
