Student Health Insurance Waiver - Upload Insurance Card Images

All BGSU Students are required to take BGSU Student Health Insurance coverage while attending BGSU.  If the BGSU Student is already covered under an alternate Health Insurance plan, the student must “Waive” the BGSU Student Health Insurance Requirement and must show proof of the alternate health insurance card information including: Insurance Company Name, Subscriber ID, Group Number, and Phone Number. We have recently enhanced the Student Health Insurance “Waiver” web application so that students can securely upload images of their insurance card (front and back) instead of manually typing the insurance information into the web application.
