Departments are strongly encouraged to use this page as a method of reducing hazards in their areas. Environmental Health and Safety may request departments to supply this information for certain types of accidents. All preparers, witnesses, and reviewers must sign the report after printing.

This report must be submitted for any injury or illness within 24 hours of occurrence.
