University Advancement forms
University Advancement (which includes the office of Advancement Services, the BGSU Alumni Association and BGSU Foundation, Inc.) is responsible for the storage, security and effective distribution of alumni and donor data. This page serves as a library of documents that BGSU faculty, staff or others in the community may need in requesting access to these types of data.

This form must be completed prior to gaining access to One, the database of alumni and donor information managed by University Advancement. - Campus training schedule

- General data request form
This form is used by anyone requesting database information from the Office of Alumni and Development. Examples include mailing lists, mailing labels, current data on graduates of a certain college or other general alumni information. It is understood that the information received is for official university business and that it will not be used for commercial purposes. Please do not release any alumni or donor information without the consent of the Alumni Records Manager. - New Gift/Pledge Form
This form should be used when making a gift, pledge or pledge payment to BGSU. It is fillable and can be returned electronically or mailed with a check payment.- Payment Information/Authorization Form
This form is only required when you are making a gift or pledge payment via credit card, EFT or employee payroll deduction. Please do not send this form electronically.

The Office of Advancement Services manages the accessibility of the constituent data and ensures that it is released in accordance with approved policy and procedure and in compliance with State and Federal laws and regulations. Employees' access to data is limited to that which is required to perform their responsibilities. All appropriate measures are taken to protect the information from unauthorized disclosure.
