Applying for Services
To apply for accessibility services, students must submit documentation of their disability for review by AS staff. There is no deadline to apply for services, but students should be advised that the review process can take several weeks. We encourage prospective students to submit their documentation as soon as they have been accepted to BGSU. Please submit documentation electronically, if at all possible.
A request for academic accommodations should include the following:
- Request for Accommodation or equivalent information. Click to initiate the process electronically and receive an automated form via BGSU email. The Request for Accommodation form is also available in pdf format for printing and manual completion if you prefer.
- Disability Verification Form or equivalent information
- Most recent IEP, 504 plan, and multi-factored evaluation (MFE), if applicable
Once we’ve received and reviewed the documentation, we will notify you in writing of your eligibility status. Notification will be sent to your BGSU email account.

We encourage contact with our office during the pre-admission process. Please schedule a phone appointment at your convenience with an Accessibility Services staff member. During that conversation, we can discuss possible academic and housing accommodations, documentation needed to apply for services, differences between services in high school and college, and other resources available to students with disabilities.
